What is your Name and Job Title?: Sharon Cassidy, Diabetes Specialist Dietitian

Please confirm that you treat those with type 1 diabetes who also have Eating disorders, including those who deliberately manipulate/ omit insulin?: yes i try to support them

What type of service are you?: I am the team leader for the diabetes specialist dietetic service for the Hull and East Rding Diabetes Network. I am also the Lead Dafne educator for the Network

Where is your service based?: My office is based at Hull Royal Infirmary - address above however the diabetes team covers potentially all of the east riding which includes Hull

Who do you care for (i.e. age limits)?: I personally work with young people in transition, young adults and beyond

What is your catchment area?: mainly Hull but we recieve referrals from all over East Riding.  we work closely with our colleagues who are based outside of Hull so if travel was a problem we would refer to them

How to get a referral?: We are commsioned to recieve referrals from either GPs or practice nurses, consultants and anyone in the specialist team
