Please confirm that you treat those with type 1 diabetes who also have Eating disorders, including those who deliberately manipulate/ omit insulin: We see patients with type 1 diabetes who also have eating disorder, however we are not specialized service for eating disorder

Where is your service based?: Chorley & South Ribble Hospital & Minerva Centre 

Who do you care for (i.e. age limits)?: Above age of 16 years 

What is your catchment area?: Central Lancashire PCT referral  

How to get a referral?: Through GP or through hospital diabetes clinic 

If self referral what is the process?:  Subjects with type 1 diabetes can contact diabetes nursing service based on above centres. 

If GP/ Diabetes team referral who should be asked for?: Diabetes Specialist Nurses 

What approach and therapies are offered?:  Counselling, combined appointment with dietician & diabetes specialist nurses, clinical psychologist
