What is a Complex Care Plan?

Our members often have multiple issues and at DWED we truly believe that all parties involved in treating them must be on the same page. We have tried to translate that literally by developing the complex care plan. The idea here is for the patient to be able to take control of their own recovery while ensuring that communications between professionals are consistent and treatment plans are harmonious. We find that one of the main barriers to long lasting recovery and effective treatment is a lack of communication between professionals and conflicting advice. The CCP aims to address this issue head on. We hope that you will help us by providing feedback on the plan, letting us know if we have missed anything or if you have an issue we can resolve.
